County Health Improvement Organizations (CHIOs) in Oklahoma are well-positioned to address social determinants of health (SDOH) in their communities. And these efforts are a great way to connect the CHIOs to the Oklahoma Primary Healthcare Improvement Cooperative (OPHIC).
