Pathways to a Healthier You for CATCH- UP 2.0

Pathways to a Healthier You- Our Story

Pathways to a Healthier You is a CHIO+ (serves individuals within multiple geographies of a minimum of 3 or more counties) and located in the Southwest Oklahoma in Jefferson and Stephens County. With a current relationship with PHIO through CATCH UP 1.0, Pathways to a Healthier You used the 2.0 grant period to expand their current work in this rural area by supplying a bridge in funding for an increased focus on planning strategies to increase community and parent voice. 

By attending the National Parent Advisory Council training through the Oklahoma Family Network and partnering with OU Health Science Center’s Parent Advisory Committee Cohort Training , Pathways identified a need to increase their community voice by establishing a Parent Advisory Council (PAC).  Pathways is forming the PAC which will be a separate committee and be accountable to the Pathways Board of Directors. Increasing the community and parent voice will lead to a stronger family-centered organization that meets the needs of the community. 

Currently, Pathways has a strong focus on NEAR Science and they utilized this background to solidify and increase relationship in this area. Through partnerships with Christian Helping Hands and Heather’s Hope, Pathways is using the trauma-informed training in the area to form a Family Resource Center (FRC). The FRC will address the social determinants of health in the area as well as increase the trauma-informed care resources available in this rural community. The PAC will help inform the types of programs offered at the FRC.

For Pathways, this grant focused on increasing community and parent voice. It allowed for compensation for parent voice and provided  a rural community with family centered and collaborative resources.

As the Executive Director, CATCH UP 2.0 funds have increased the community and parent voice in my area, creating a stronger organization.

The most impactful part of the CATCH-UP 2.0 is the ability to expand our community voice in a rural community.
