FAQs
The information sheet provides valuable information about testing. In addition, for individuals who are unable to register (by self or site) in the OK COVID-19 Portal, this sheet provides a space for them to write down their code word to receive results via phone call.
The survey cards will be provided to you. Each one contains a unique link/QR code for individuals to use to access the survey. A member of the CATCH-UP team will bring them or ship them to your testing event.

However, the system generates a printable PDF so most printers should be able to be configured to print the correct size label.
